Risk factors for surgical site infections after caesarean section at Yaounde, Cameroon

Background: Surgical site infection is the invasion by microorganisms of the tissue layers affected by the surgical procedure. Maternal morbidity from infections has been shown to be higher after caesarean section compared to the vaginal delivery. Objective of the research was to analyze the risk factors associated with surgical site infections after caesarean section.Methods: This was a cross sectional (affected/non affected) study approved by the institutional committee for ethics and research of the faculty of medicine and biomedical sciences. A total of 310 medical files were assessed, 62 files from patients with surgical site infections and 248 files from patients without any complications. The data was collected using a pretested questionnaire and analyzed using the statistical package for the social sciences (SPSS) software version 22.0. The Chi squared and the Fisher exact tests were used to assess homogeneity between the 2 groups. Odd ratio 95% confidence interval was used to assess the association between the variables.Results: The proportion of surgical site infections during the study was 1.81%. Factors associated with surgical site infections were premature rupture of membranes (OR: 2.065; 95% CI 1.051-4.05; p=0.035); the vertical midline incision (OR=5.26; 95% CI; 1.41-19.57; p=0.013) and a operation by a resident physician doctor (OR=1.98; 95% CI 1.09-3.59; p=0.02).Conclusions: A factors associated with surgical site infections after caesarean section are a premature rupture of membranes, vertical midline incision and the qualification of the practitioner

Socio-environmental analysis of climate dynamics as experienced by the population living near the Masako Forest Reserve

Une étude sur l’analyse socio-environnementale des indicateurs de la dynamique du climat vécus par la population riveraine de la Réserve Forestière de Masako a été réalisée dans le village Masako. 60 ménages ont été enquêtés sur base d’un questionnaire pré-élaboré. Les résultats révèlent qu’au niveau du village Masako, la population perçoit les changements climatiques à travers les indicateurs tels que les pluies précoces ou tardives, les vents violents, les chaleurs élevées, les érosions et les perturbations des saisons. L’impact le plus important de ces changements climatiques est perçu au niveau de l’agriculture où les cultivateurs évoquent le flétrissement des cultures par manque d’eau, la difficulté du brûlis pour des précipitations précoces et la prolifération des insectes ravageurs qui se traduit par une baisse de la production agricole affectant l’insécurité alimentaire des ménages. Mais d’autres secteurs comme celui de la pêche, la carbonisation et le petit commerce ne sont pas épargnés. L’ensemble de la population enquêtée note que les perturbations climatiques observées dans le milieu présentent des impacts négatifs sur les ressources de la Réserve Forestière de Masako tels que la diminution des produits forestiers autre que le bois (gibiers, chenilles, escargots, etc.). The results reveal that at the Masako village level, the population perceives climate change through indicators such as early and late rains, high winds, high temperatures, erosions and seasonal disturbances. The most important impact of these climate changes is seen in agriculture where farmers refer to crops wilting for lack of water, the difficulty of burning at early rainfall and the proliferation of insect pests that result in a decline in agricultural production. Other sectors such as fishing, charcoal production and small scale trading are not spared. The entire surveyed population revealed that the climatic disturbances observed in their environment have negative impacts on the resources of the Masako Forest Reserve, such as the reduction of forest products other than wood (game, caterpillars, snails, etc.). Loss-of-Function Mutations in LRRC6, a Gene Essential for Proper Axonemal Assembly of Inner and Outer Dynein Arms, Cause Primary Ciliary Dyskinesia

Primary ciliary dyskinesia (PCD) is a group of autosomal-recessive disorders resulting from cilia and sperm-flagella defects, which lead to respiratory infections and male infertility. Most implicated genes encode structural proteins that participate in the composition of axonemal components, such as dynein arms (DAs), that are essential for ciliary and flagellar movements; they explain the pathology in fewer than half of the affected individuals. We undertook this study to further understand the pathogenesis of PCD due to the absence of both DAs. We identified, via homozygosity mapping, an early frameshift in LRRC6, a gene that encodes a leucine-rich-repeat (LRR)-containing protein. Subsequent analyses of this gene mainly expressed in testis and respiratory cells identified biallelic mutations in several independent individuals. The situs inversus observed in two of them supports a key role for LRRC6 in embryonic nodal cilia. Study of native LRRC6 in airway epithelial cells revealed that it localizes to the cytoplasm and within cilia, whereas it is absent from cells with loss-of-function mutations, in which DA protein markers are also missing. These results are consistent with the transmission-electron-microscopy data showing the absence of both DAs in cilia or flagella from individuals with LRRC6 mutations. In spite of structural and functional similarities between LRRC6 and DNAAF1, another LRR-containing protein involved in the same PCD phenotype, the two proteins are not redundant. The evolutionarily conserved LRRC6, therefore, emerges as an additional player in DA assembly, a process that is essential for proper axoneme building and that appears to be much more complex than was previously thought

La localisation de la Gergovia antique et des ouvrages du siège césarien est admise depuis plusieurs années, au moins au sein de la communauté scientifique. Une mission d’expertise conduite au milieu des années 1990 a en effet permis de valider le tracé d’une partie des fortifications tel qu’il avait été décrit à l’issue des recherches du Second Empire. La multiplication des interventions archéologiques dans ce secteur du bassin clermontois permet de préciser le tracé du dispositif militaire installé en 52 av. J.-C. La mise au jour de nouveaux vestiges immobiliers (fossés, sépulture humaine et ensevelissements de chevaux, tronçons de voies) et mobiliers (militaria tardo-républicains) permet en effet de mieux cerner l’emprise du champ de bataille. Elle conduit à formuler des hypothèses nouvelles quant au déroulement du siège de l’oppidum.The location of the ancient Gergovia and the Caesarian siege works has been known for several years, at least within the scientific community.
